The eReferral to LTCH Referral process involves the placement of clients in Long Term Care Homes (LTCH). This process allows for accurate communication of information between Ontario Health at Home (OHaH) and LTC Homes. It also provides an electronic record of all major events encountered in the process of placing an OHaH client into a LTCH. Also note that while HPG still uses Ontario Health at Home’s former ‘LHIN’ name, the Reference Guides generally use the current ‘OHAH’ name, but will use the term ‘LHIN’ where it applies specifically to a label in the HPG system.
